非侵入性神經(jīng)調(diào)控技術(shù)是一種潛力巨大的非藥物鎮(zhèn)痛手段，具有經(jīng)濟(jì)、易操作、安全性高等優(yōu)點(diǎn)，已被應(yīng)用于各類臨床疼痛的治療。然而，目前仍缺乏對不同神經(jīng)調(diào)控技術(shù)鎮(zhèn)痛特性的深入理解。因此，本文從鎮(zhèn)痛效果和鎮(zhèn)痛機(jī)制兩個方面入手，評述非侵入性外周神經(jīng)電刺激(經(jīng)皮神經(jīng)電刺激、經(jīng)皮迷走神經(jīng)電刺激)和中樞神經(jīng)電刺激(經(jīng)顱直流電刺激、經(jīng)顱交流電刺激)在鎮(zhèn)痛方面的研究結(jié)果，總結(jié)各技術(shù)對急性疼痛和慢性疼痛的常用刺激參數(shù)和鎮(zhèn)痛效果，探討可能的鎮(zhèn)痛機(jī)制。最后，本文對比和總結(jié)各技術(shù)的鎮(zhèn)痛特點(diǎn)，討論了現(xiàn)有研究的若干局限和未來的研究方向?？朔@些局限將促進(jìn)相關(guān)技術(shù)的臨床應(yīng)用，最終達(dá)到幫助患者緩解疼痛的目的，減輕疼痛對患者、其家庭和整個社會帶來的健康和經(jīng)濟(jì)負(fù)擔(dān)。

Other Abstract

As non-pharmaceutical interventions, non-invasive neuromodulation techniques are promising in pain management. With many advantages, such as low costs, high usability, and non-invasiveness, they have been exploited to treat multiple clinical pain. Proper use of these techniques requires a comprehensive understanding of how they work. In this article, we reviewed recent studies concerning non-invasive electrical peripheral nerve stimulation (transcutaneous electrical nerve stimulation and transcutaneous vagus/vagal nerve stimulation) as well as electrical central nerve stimulation (transcranial direct current stimulation and transcranial alternating current stimulation). Specifically, we discussed the oft-used stimulation parameters, the analgesic effects on acute and chronic pain, and the neural mechanisms underlying the analgesic effect of each technique. We then contrasted the four kinds of nerve stimulation techniques, pointing out limitations of existing studies and proposing directions for future research. With more extensive and in-depth research to overcome these limitations, we shall witness more clinical applications of non-invasive electrical nerve stimulations to alleviate patients’ pain and ease the crippling medical and economic burden imposed on patients, their families, and the entire society.
