Objective:To explore and analyze the effect of chronic disease management in community on standardized medication in patients with hypertension and provide basis for clinical treatment.Method:A total of 150 hypertensive patients admitted to Ningde Municipa Hospital general base collaborative unit of Jiaonan Community Health Service Center from February 2016 to February 2017 were selected as the objects of study and randomly divided into the observation group and the control group,75 cases in each group.The control group received routine blood pressure control treatment.On this basis,the observation group was given chronic disease management on standardized medication.The control of blood pressure,the occurrence of complications and the quality of life of the two groups were compared.Result:After treatment,Blood pressure in both groups were decreased,and the blood pressure in the observation group were significantly better than those in the control group,the differences were statistically significant(P<0.01).The blood pressure control rate in the observation group was 93.33%,which was significantly higher than 66.67% in the control group,the difference was statistically significant(P<0.01).Compared with the control group,the complication rate was lower,and the quality of life scores were higher in the observation group,the differences were statistically significant(P<0.01).Conclusion:Chronic disease management for patients with hypertension in community has a significant curative effect,it can effectively control the blood pressure,reduce the occurrence of complications,and improve the quality of life by standardized medication guidance and health intervention,and is worthy of further promotion and application.
