Objective:To investigate the effect of family doctor contract service and chronic disease management on hypertensioncontrol in community patients. Methods:From Oct 2016 to Sep 2017,300 patients with hypertension were randomly divided into 3 groups,control group(100 cases),family doctor contract group(100 cases)and chronic disease management group(100 cases).According to the blood pressure classification adopted in China,hypertension patients in the 3 groups were further divided into 3 groups. The control group was treated with routine community intervention therapy and health management. Based on the interventionof the control group,the family doctor contract group received family doctor contract service to provide high quality,safe and effectivebasic medical and public health services,and the chronic disease management group received the standardized management ofchronic diseases. After 6 months,the systolic and diastolic blood pressure(SBP and DBP),exercise volume,compliance behavior,lifestyle(smoking,alcoholism,high-salt and high-fat diet),drug compliance,emotional control,and cardiovascular andcerebrovascular complications were observed and analyzed in the 3 groups. Results:After 6 months of intervention,the SBP andDBP in the family doctor contract group and chronic disease management group were all lower than those in the control group(P <0.05). The emotional control,exercise volume,drug compliance,compliance behavior and lifestyle in the family doctor contractgroup and chronic disease management group were all better than those in the control group(P<0.05). Conclusion:The familydoctor contract service and chronic disease management can effectively control the hypertension of community patients,change thelife style and improve drug compliance. The chronic disease management team performs the health management and follow-up tocomprehensively understand the actual situation of hypertension patients,and to give individualized guidance and suggestions. It ishelpful to improve the understanding and treatment compliance of patients with hypertension,reduce the risk of exacerbation ofhypertension patients,prevent complications,and improve the quality of life of patients.
